This toolkit was developed by ChangeLab Solutions, the Centers for Disease Control and Prevention (CDC), the National Alliance of State Pharmacy Associations (NASPA) and the American Pharmacists Association (APhA) as part of a cooperation agreement with ChangeLab Solutions. The resource helps pharmacists and prescribers navigate the CPA`s network of requirements and adapt a CPA model to best meet their individual clinical, legal and administrative conditions. Models of practice agreements based on cooperation help pharmacists develop their pharmacies. Note: These models comply with state laws and regulations, such as rehabilitation. The resource contains a customizable CPA model and national details national laws may differ as to whether CPAs apply to individual or multiple patients, are limited to certain practice parameters, in which they are allowed to enter (. B for example, all prescription doctors, doctors, doctors and nurses only), pharmacist qualifications (for example. B approved or registered by the state boards of directors, PharmD, continuing education) and much more. Some states require parties to take out liability insurance or an invalid rebate after a period of time. An agreement on collaborative practice is a document signed by a certified physician that authorizes a pharmacy to offer specific services to patients without individual prescriptions. Collaborative Practice Agreements (CPAs) create formal practical relationships between pharmacists and prescribers.
CPAs can use collaborative care by defining functions that, in addition to the typical pharmacist`s practice, are delegated to the pharmacist by the co-operative prescriber under negotiated terms defined in the agreement. A new CDC Department for Heart Disease and Stroke Prevention (DHDSP) describes the launch steps and practical elements of the implementation of cooperation agreements (CPAs). CPAs, which formalize practical relationships between physicians and pharmacists, are an increasingly common opportunity to integrate pharmacists into team-based care. „Advancing Team-Based Care Through Collaborative Practice Agreements: A Resource and Implementation Guide for Adding Pharmacists to the Care Team“ contains a summary of the laws governing CPAs in all 50 states and the District of Columbia, and contains a customizable CPA model. The Government Opoperative Committee for the Promotion of Practice and Innovation (formerly the Therapy Drug Management Committee) has developed models of cooperation agreements (CPAs) as an advantage for takeover members that they can use in their offices. These models contain a CPA model and schedules for certain disease conditions that may appear in a CPA. There is also an appendix for additional disease conditions that pharmacists wish to include in a CPA. These documents are available in Word format, so they can be adapted to certain exercise settings.
Pharmacists who wish to develop an agreement on a community practice may need help determining where to start. To support this process, a toolkit entitled „Advancing Team-Based Care through Collaborative Practice Agreements“ has been developed. The toolkit is a resource for pharmacists that they can use in the development and implementation of collaborative practical agreements in a team-based spirit of care development.
